Two simple steps

I’ve tried to make my booking process as seamless as possible but if at any point you have questions, please don’t hesitate to reach out to me at laura@kaleidoscopewellness.ca.

Step 1

If you’re new to my practice, there are two forms we will need to complete during our first session together.

  1. Consent form: we will need to go through this form together when we first meet as I will require your signed consent to provide my services.

  2. Intake form: you can review and complete this form before our first session together to save some time but if you prefer to complete it during our session that works too.

Step 2

Next, simply scroll down and use the scheduling tool to book a day and time that works best for you. It syncs with my calendar so it’s the fastest way to book your session. The scheduling tool will then send you a confirmation email with the details of your session. Then, 24 hours before your session, you’ll receive a reminder email. That’s it!
